The Community Health Foundation of Western and Central New York?
awarded new grants totaling more than $200,000 in the second half of 2011.The grants focused on three main areas: improving the quality of care for frail elders, improving health care for young children living in poverty, and growing community health capacity.
In the area of elders, the following grants were awarded:
- HealtheLink of Buffalo, $93,750 to support care coordination in partnership with Catholic Medical Partners for seniors with major depression
- P2 Collaborative of WNY Inc., $30,743 to build a collaborative effort in rural western New York to improve care transitions
Grants to support young children living in poverty were awarded to:
- Children?s Dental Health Project of Washington, D.C., $49,900 for the Children?s Dental Health Project, in partnership with the Centers for Disease Control and Prevention and the New York State Department of Health?s Bureau of Dental Health
- UB Regional Institute, $12,500 to conduct research and complete a research brief on childhood trauma in Central New York in conjunction with a speaker series
Three grants were awarded to help grow community health capacity:
- P2 Collaborative, $7,000 to enable visits to three health equity centers by the Health Equity Coalition and Healthy Lifestyles Coalition
- East Hill Family Medical Inc. of Auburn, $5,000 to help develop an application for a new Federally Qualified Health Center
- Clean Air Coalition of WNY, $5,000 to support a strategic planning process
